If you would like to register your child at P.S. 76, please bring the following: ​​

Proof of Address: 
Two proofs of address must be provided. Current copies of the  following documents may be used as proof of address. Any two of the documents below will be accepted as proof.

• ​Gas or electric bill in parent's or guardian's name. Document or letter from a federal, state or  local government agency, including the  IRS, Housing Authority, Human Resources Administration, Administration for Children's Services, and US Citizenship  and Immigration Services.

• An original lease agreement, deed or mortgage statement for the residence

• A property tax bill for the residence

• A water bill for the residence

• Official payroll documents issued within the last 60 days. A letter form an employer is not acceptable.

• Proof of Age: Original Birth Certificate, Baptismal Certificate, Passport or Immigration Record.

• Proof of Immunization: From Pediatrician or CIR (Central Immunization Registry)

The child you are registering MUST come with you.
